A national healthcare app designed to empower Singaporeans to lead healthier lives by encouraging vaccination adoption, promoting better diet and exercise habits, and supporting smoking cessation.

The current national healthcare app provides residents over 40 with a digital health plan after consultations with their doctor. However, feedback showed that Singaporeans wanted more personalised recommendations, clearer prescriptions, and better tools to support and build healthier habits.
To address this, we collaborated with government agencies and a strategy consulting firm to redesign the app experience—making it more engaging and user-friendly to drive citizen adoption, improve adherence to the health plan provided by their doctor, and ultimately improve public health.
Identify mindsets, motivations and barriers for adoption of digital health plan, working closely with clinicians and residents.
Drive improved adherence through tools, trackers and dashboards that drives behaviour change of the citizens towards a self-motivated and preventative approach.
Improve health outcomes and reduce reliance on healthcare services in the long term.
The team reviewed existing data on the health ecosystem and conducted up to 3 concept testing with wireframes to validate and enhance previous research. Additionally, we audited digital products and industry best practices to inform our approach and drive adoption.
Through concept testing, we gained deeper insights into the pain points and attitudes of both residents and clinicians regarding vaccinations, lifestyle prescriptions, and smoking cessation.
Using these findings, we developed the Health Plan concept to be anchored by an action-driven approach, designed to effectively address these challenges and encourage long-term behaviour change.
Our designs needed to align with the existing look and feel of the HealthHub app while seamlessly integrating the new features. By referencing existing screens, we ensured visual consistency, focusing primarily on refining card-based layouts to enhance usability and maintain a cohesive user experience.
After developing the designs for each scope area, we conducted usability testing to further refine our designs, ensuring it effectively met the needs of residents.
